Plants included: Exact species will vary based on availability but will include at least 8 flowers and 1 grass for full sun and medium moisture, with heights of 3 feet or less. Example mix:

How to plant: For a meadow look, plant the grasses evenly throughout the space and scatter flowers informally. Plant densely: about 12 inches from center to center, keeping all plants at least 8 inches from the edge of the bed. Covers about 30 square feet.
Replacing lawn? For best results, remove existing vegetation before planting. I recommend smothering with cardboard and mulch for 2-3 months during the growing season. You can then push the mulch aside and easily plant through the cardboard.
